Ken,

The surface patch is for closed structures only, such as a
box. It is easily confused with "patch" as in patch
antenna, but it can't be used for a patch antenna because a
patch ;antenna is open all around. I presume that a person
wanting to model a patch antenna could do so using a wire
grid just above a ground plane, with the ground plane having
the properties of the desired substrate. I haven't tried
this myself.
