NEC-LIST: Pointers to answers for various questions?

I would like to know (or know where I can find the answers to):

What are the differences between nec2d, nec4* etc?

Where can I find the grammar for the output file formats (obviously
        dependent on which cards are in use)?

Is there a clearer description of the diagnostics anywhere -- if nec2d
        reads a real and thw program writing the cards has not printed
        the ".0000" then nec2d fails, but the message (I forget what now)
        is a bit obscure.
